PAYMENT POLICY
Physical therapy services are provided and billed by CORE Rehabilitation, LLC. Our providers participate in all major health insurance plans in the region. Below are the health plans which we currently accept in-network. If your plan is not listed, please call our office so that we can discuss your options as some insurances allow out-of-network services.
Please be aware that with any health plan, there may be deductibles and co-payments for which you could be responsible.

QUESTIONS ABOUT YOUR BILL
What billing or insurance information will I receive?
​You will receive an invoice from CORE Rehabilitation, LLC after you have been discharged from physical therapy if you have a remaining balance. You will never receive a request for payment from any 3rd party company.
​
You will also receive an Explanation of Benefits from your insurance company as your claims are processed.
​
How long will it take to get things settled with the insurance company?
Generally, it takes from 45 to 60 days to obtain payment from an insurance carrier.​
​
How much will I owe each visit?​
​All plans are different and usually require each insured party to be responsible for a co-pay or co-insurance each visit. Some plans are subject to the deductible, while others are not. We do our best to let you know up front what your insurance requires so there are no surprises or hidden costs.
